The Endependence Center is collecting information about the experiences of people with disabilities when applying for housing. This survey is optional and will not affect any services you receive from ECI.
We may want to ask you follow-up questions about your experiences. Sharing your name and contact information is not required, but it is helpful. We will not share any personal information with anyone outside of ECI. If you need help with a specific housing issue, please speak directly with an ECI staff member.
